Post Rehab is seeking a Physical Therapist Assistant (PTA) to join the rehabilitation team at Civita Care Center at Sheridan Woods. This role offers the opportunity to provide hands-on therapy while helping residents build strength, improve mobility, and work toward greater independence in a skilled nursing and post-acute care environment.

What You'll Do

• Provide physical therapy treatments under the direction of a licensed Physical Therapist
• Assist residents with therapeutic exercises, gait training, balance, transfers, and functional mobility
• Carry out individualized treatment plans based on resident needs and rehabilitation goals
• Monitor resident response to treatment and communicate progress to the supervising PT
• Reinforce exercises, mobility techniques, and safety strategies with residents and caregivers
• Maintain accurate and timely clinical documentation
• Collaborate with PT, OT, SLP, nursing, and other members of the interdisciplinary care team

Qualifications

• Active Connecticut PTA license required
• Strong communication and teamwork skills
• Skilled nursing or rehabilitation experience preferred, but not required
• New graduate PTAs are welcome to apply
