Autumn House Bloomfield Hills | Bloomfield Hills, Michigan

Every Other Weekend + Holidays | Shared On-Call Responsibility

Afternoon Shift: 3:00 PM to 11:00 PM

Lead the shift. Support the team. Protect the standard of care.

Autumn House Bloomfield Hills, a Cardinal Senior Living community, is looking for a Lead Care Partner who knows how to step up when the shift gets busy.

This is not just another caregiving position.

We’re looking for someone who can provide great resident care while also helping lead the people delivering it.

You’ll be the person Care Aides look to for direction, the person who notices when something is not getting done, and the person who helps keep the shift organized when priorities change.

If you like being the one who can calm the room, solve the problem, and get the team moving in the same direction, this role may be exactly what you’re looking for.

You’ll probably love this role if you like to:

  • Lead from the floor instead of from behind a desk

  • Help teammates perform at a higher level

  • Make decisions when things do not go according to plan

  • Keep resident care organized and on track

  • Coach people directly when expectations are not being met

  • Notice gaps before they become bigger problems

  • Jump in and help instead of saying, “That’s not my job”

  • Keep communication clear between shifts and departments

  • Be trusted with real responsibility

  • Finish the shift knowing the residents and team were better because you were there

What You’ll Own

As Lead Care Partner, you’ll help oversee the day-to-day execution of resident care during your shift.

You’ll:

  • Provide direction and support to Care Aides

  • Help ensure residents receive safe, respectful, and consistent care

  • Monitor rounds, assignments, and completion of resident-care responsibilities

  • Review communication logs and ensure appropriate follow-through

  • Help maintain Care Aide assignments, grids, and staffing coverage

  • Support onboarding and training of new team members

  • Coach staff on expectations, documentation, and resident-care standards

  • Identify staffing or operational problems and help develop solutions

  • Monitor care and medication-related documentation for completion and accuracy

  • Escalate resident-care concerns, staffing issues, and operational risks appropriately

  • Assist with emergencies and changes in resident condition

  • Step in as the shift leader when other community leadership is unavailable

  • Participate in shared on-call responsibilities with the leadership team

  • Provide direct resident care whenever the team needs support

This Is a Working Leadership Role

We are not looking for someone who wants to stand back and supervise.

The Lead Care Partner works alongside the team.

There will be times when you’re coaching an employee, solving a scheduling issue, checking documentation, helping with a resident transfer, responding to a concern, and making sure the next shift has what it needs.

Strong leadership in this role means seeing what needs to happen and helping make it happen.

What We’re Looking For

We’re looking for someone who is dependable, calm under pressure, organized, compassionate, and comfortable holding people accountable.

Required:

  • High school diploma or GED

  • At least 18 years old

  • 1+ year of leadership or management experience in healthcare

  • Strong communication and organizational skills

  • Ability to provide clear direction and feedback

  • Ability to assist residents with mobility and personal care

  • Ability to lift up to 50 pounds occasionally

  • Willingness to work every other weekend and required holidays

  • Willingness to participate in shared on-call responsibilities

  • CPR, First Aid, and AED certification or willingness to obtain

Preferred:

  • CNA or Medical Assistant background

  • Senior living or long-term care experience

  • Previous experience leading Care Aides, CNAs, caregivers, or direct-care staff

  • Experience with staffing, scheduling, documentation, and resident-care coordination
