Stern Therapy Consultants is adding a PRN Physical Therapist Assistant to the rehabilitation team at Autumn Lake at Southgate. Help skilled nursing residents gain confidence with everyday movement through focused treatment, encouragement, and close coordination with the supervising Physical Therapist.

What You'll Do

• Deliver prescribed interventions that support the mobility goals in each resident's PT plan of care.

• Lead therapeutic exercises and functional activities addressing strength, balance, and endurance.

• Guide residents through walking, bed mobility, and transfers using appropriate assistance and equipment.

• Reinforce safe movement habits and instructions for prescribed mobility devices.

• Observe residents' responses, recognize changes in function or tolerance, and report concerns to the PT.

• Maintain accurate treatment records and share mobility recommendations with nursing and rehabilitation colleagues.

Qualifications

• Active New Jersey Physical Therapist Assistant license.

• Ability to provide individualized treatment within an established plan of care.

• Strong communication and timely clinical documentation.

• Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.
