About the Opportunity

We are seeking a Physical Therapist Assistant to join a supportive skilled nursing facility (SNF) rehabilitation team in Santa Rosa, CA. Working under the supervision of a licensed Physical Therapist, you will carry out individualized treatment plans and help patients progress toward meaningful functional goals.

Responsibilities

·        Implement treatment plans established by the supervising Physical Therapist.

·        Provide therapeutic exercise, gait, balance, mobility, and functional training.

·        Monitor patient response and communicate progress to the Physical Therapist.

·        Educate patients and caregivers within the established plan of care.

·        Complete accurate and timely documentation.

·        Collaborate with the rehabilitation and interdisciplinary care team.

Qualifications

·        Graduate of an accredited Physical Therapist Assistant program.

·        Active CA PTA license in good standing, or eligibility for licensures.

·        Current BLS/CPR certification as required by the employer.

·        Strong communication, documentation, teamwork, and patient-care skills.

·        Experience in the applicable setting preferred.
