We have a new Administrator and an exciting opportunity for a compassionate, motivated Social Services Manager to join our team! If you have a bachelor's degree and experience in long-term care, we want to hear from you.

Responsibilities

• Provide support and advocacy for resudents and their families.
• Coordinate psychosocial assessments and individualized care plans.
• Assist with discharge planning, referrals, and community resources.
• Participate in interdisciplinary care plan meetings.
• Promote resident rights, dignity, and quality of life. 
• Maintain accurate documentation and suport regulatory compliance. 

Qualifications

• Bachelor's degree required in Social Work, Human Services, Psychology, Sociology, or a related field. 
• Experience in long-term care or skilled nursing facility required. 
• Strong communication, organizational, and interpersonal skills.
• Knowledge of resident rights and discharge planning. 
• Social Woeker license in North Carolina a plue (LBSW, LSW, LCSW, LMSW, etc.)
