Bring your approach to recovery to Civita Care Center West River. Post Rehabilitation Services is seeking a full-time Physical Therapist to help skilled nursing residents rebuild physical abilities, overcome mobility barriers, and prepare for their next stage of care.

What You'll Do

• Assess residents' movement, strength, balance, and functional abilities to identify treatment priorities.

• Design individualized plans of care that account for medical precautions, previous function, and resident goals.

• Provide therapeutic exercise, gait training, and task-specific practice for transfers and daily mobility.

• Evaluate assistive device needs and train residents and caregivers in safe movement strategies.

• Measure progress, update treatment plans, and provide clinical direction to PTAs.

• Complete evaluations and treatment records while partnering with nursing and rehabilitation colleagues on discharge recommendations.

Qualifications

• Active Connecticut Physical Therapist license.

• Strong clinical judgment, assessment, and treatment planning skills.

• Effective communication and timely, accurate documentation.

• Skilled nursing or post-acute experience is a plus; new graduates are welcome to apply.
