**Shift Information:**
- 12-hour night shift from 7 PM to 7 AM every four weeks starting, and then it will change to every other week in December.
- **Weekend Work:** Yes
**Requirements:**
- **Board Certified:** Must be certified.
- **BLS and ACLS:** Must have these certifications.
- **DEA:** Need verbal confirmation at the time of name clearance.
- **Clean Background:** Important for quick approval (if there are any issues, the provider will be redirected).
- **Michigan License:** Must have it ready.
**Employee Health Requirements:**
- **Flu Vaccine:** Must have for the 2024-2025 season.
- **COVID Documentation:** If the 2024-2025 booster is not received, a declination is needed.
- **TB Testing:** Two-step skin test within the last 12 months or Quantiferon within the last 6 months.
- **TB Questionnaire:** Must complete it.
- **Hep B:** Need a titer with numbers or a declination.
- **MMR:** Vaccine series or titer with numbers.
- **Varicella:** Vaccine series or titer with numbers.
- **TDAP:** Adult vaccine within the last 10 years.
**Certification and License Requirements:**
- Must be Board Certified.
- Must have a license in Michigan.
