Ultimate Therapy is hiring a full-time Physical Therapist at Autumn Lake at Glen Hill. Work with skilled nursing residents at different stages of recovery, translating your clinical findings into treatment that helps them move safely and participate more independently in daily life.

What You'll Do

• Assess walking, balance, strength, range of motion, and transfer abilities to identify rehabilitation needs.

• Set measurable goals that reflect residents' prior function, medical precautions, and personal priorities.

• Deliver individualized therapeutic exercise, gait training, and functional mobility treatment.

• Determine appropriate mobility equipment and teach residents and caregivers how to use it safely.

• Reevaluate progress, modify plans of care, and provide clinical supervision to PTAs.

• Complete accurate clinical records and coordinate with nursing and rehabilitation colleagues on ongoing care and discharge recommendations.

Qualifications

• Active Connecticut Physical Therapist license.

• Strong evaluation, clinical reasoning, and treatment planning skills.

• Effective communication and timely documentation.

• Skilled nursing or post-acute experience is helpful; new graduates are welcome to apply.
