Ultimate Therapy is seeking a full-time Speech-Language Pathologist at Autumn Lake at Crystal Springs. Help skilled nursing residents stay involved in conversations, communicate care needs, and manage cognitive and swallowing challenges through treatment built around their daily lives.

What You'll Do

• Evaluate speech, language, cognitive-communication, and swallowing abilities to establish treatment priorities.

• Develop measurable goals that reflect residents' clinical needs and personal preferences.

• Provide individualized therapy for communication, memory, attention, and functional problem-solving.

• Deliver swallowing interventions and coordinate recommendations with nursing and dietary staff.

• Teach residents, families, and caregivers practical strategies to reinforce progress outside therapy.

• Reassess outcomes, maintain clinical documentation, and contribute to interdisciplinary care and discharge planning.

Qualifications

• Active West Virginia Speech-Language Pathologist license.

• Clear communication and accurate, timely documentation.

• Skilled nursing or geriatric rehabilitation experience is a plus.
