Perfected Health Care is seeking a Licensed Practical Nurse (LPN) to provide attentive, professional nursing care to patients in their homes. This role is associated with our Moorestown, New Jersey location and emphasizes patient safety, accurate documentation, and coordinated care.

Responsibilities
• Provide home health nursing care according to the individualized plan of care, authorized orders, and appropriate clinical supervision.
• Administer medications and treatments as ordered, within the New Jersey LPN scope of practice and agency policies.
• Monitor patient condition, vital signs, symptoms, and responses to medications or treatments; promptly report changes or concerns to the supervising registered nurse and appropriate care team members.
• Maintain accurate, timely documentation of nursing care, medication administration, patient observations, and care team communications.
• Communicate with the supervising nurse, providers, caregivers, and other care team members to support continuity of care.
• Reinforce patient and caregiver instructions consistent with the care plan and nursing direction.
• Follow infection prevention practices, protect patient confidentiality, and maintain a safe care environment.

Qualifications
• Active New Jersey Licensed Practical Nurse (LPN) license required.
• Knowledge of safe medication administration, patient monitoring, and nursing documentation.
• Ability to follow care plans and clinical instructions while recognizing and reporting changes in patient condition.
• Clear communication, sound judgment, dependability, and a compassionate approach to home-based care.
• Commitment to practicing within the LPN scope of practice and maintaining professional standards.
