Perfected Health Care is seeking a Certified Home Health Aide (CHHA) to provide compassionate, respectful care to clients in their homes. This role is associated with our Moorestown, New Jersey location and supports clients in maintaining comfort, dignity, and independence.

Responsibilities
• Provide personal care according to each client’s care plan and supervising nurse’s instructions.
• Assist with bathing, dressing, grooming, toileting, and other activities of daily living while protecting privacy and dignity.
• Assist with safe mobility, walking, positioning, and transfers using approved techniques.
• Offer companionship, conversation, and meaningful engagement.
• Observe changes in the client’s condition or needs and promptly report concerns to the supervising nurse or care team.
• Complete accurate, timely documentation of care provided and relevant observations.
• Follow infection prevention practices, maintain a safe care environment, and respect client confidentiality.

Qualifications
• Current New Jersey Certified Homemaker-Home Health Aide (CHHA) certification required.
• Ability to follow individualized care plans and work within the CHHA role.
• Compassion, dependability, and clear communication skills.
• Ability to assist safely with personal care and mobility needs.
• Commitment to accurate documentation, professional conduct, and respectful client relationships.
